MedRebuttal provides a letter generation tool that assists patients in preparing written internal appeal letters for denied health insurance claims. This service does not constitute legal advice, medical advice, or professional representation of any kind. No attorney-client relationship, patient advocate relationship, or any other professional relationship is created by your use of this service. MedRebuttal is not a law firm, is not a licensed attorney, is not a healthcare organization, and is not a patient advocate organization.

The letters generated by this tool are based on common legal frameworks, federal statutes including the Affordable Care Act, and established principles of health insurance appeals. They are not tailored legal documents prepared by an attorney with knowledge of your specific circumstances, policy language, or applicable state regulations. For legal guidance specific to your situation, you should consult a licensed healthcare attorney or a licensed patient advocate in your state.

2. No Guarantee of Outcome

MedRebuttal makes no representation or warranty — express or implied — that use of a generated appeal letter will result in approval, payment, reversal, or any specific outcome from your insurance company, employer plan, or any reviewing body.

The outcome of any health insurance appeal depends entirely on the specific facts of your claim, the language of your specific health insurance plan, your insurer's internal policies and procedures, applicable state and federal law, the evidence you submit with your appeal, and the discretion of the reviewing party. Results will vary significantly across individuals, insurers, states, denial types, and clinical circumstances. MedRebuttal cannot predict, influence, or guarantee the outcome of your appeal.

The appeals success statistics referenced on this site reflect aggregate outcomes across large populations of health insurance appeals and are sourced from published research. They do not represent a guarantee or prediction of outcome in your individual case.

3. Appeal Deadlines Are Your Responsibility

Health insurance internal appeals are subject to strict deadlines — typically 180 days from the date of the denial notice under ACA-compliant plans, though some plans and denial types may have shorter deadlines. It is your sole responsibility to identify the appeal deadline applicable to your specific denial and to submit your appeal before that deadline. MedRebuttal does not monitor your deadlines, does not send reminders, and is not responsible for any consequence of a missed appeal deadline. Your denial notice must include the applicable deadline and submission instructions by law — review it carefully before using this service.

4. Accuracy of Information

You are solely responsible for the accuracy, completeness, and truthfulness of all information you enter into this tool. The quality and relevance of your generated letter depends entirely on the accuracy of the information you provide. MedRebuttal is not responsible for errors, omissions, inaccuracies, or adverse outcomes resulting from incorrect or incomplete information entered by you. Do not submit an appeal letter containing information you know to be false or misleading.

5. Scope of Coverage

MedRebuttal currently covers five denial categories: Prior Authorization, Medical Necessity, Out-of-Network and Provider Not Covered, Administrative Denial, and Benefit Limit Reached. Within each category, five specific denial reasons and three rebuttal approaches are available. Not all denial types, insurer-specific processes, plan types, or clinical circumstances are covered by this tool. It is your responsibility to verify that your specific denial reason is addressed by this service before purchasing. All available denial categories and reasons are displayed before payment is required.

6. One-Time Purchase

Each purchase grants you the ability to generate and download one appeal letter for one denied claim. Access is personal and non-transferable. You may not share, resell, or transfer your purchased access to another person.

7. Refund Policy

All sales are final. MedRebuttal does not issue refunds. Your appeal letter is generated and made available for download immediately upon payment confirmation. Because the digital product is delivered instantly and in full at the time of purchase, no refund is available after payment is processed — including if you change your mind, if the letter does not produce your desired outcome, if you entered incorrect information, or if your appeal is unsuccessful. Please review all form entries carefully on the Review step before completing payment. If you experience a technical issue preventing you from accessing your letter after payment, contact us at support@medrebuttal.com and we will resolve it.

8. Intellectual Property

The letter templates, rebuttal language, site design, content, and all intellectual property on this site are the property of MedRebuttal. Your individually generated letter is yours to use for your own personal health insurance appeal. You may not resell, reproduce, publish, distribute, or use the template language, rebuttal arguments, or site content for any commercial purpose without written permission.

9. Limitation of Liability

To the maximum extent permitted by applicable law, MedRebuttal's total liability to you for any claim arising from or related to your use of this service — regardless of the cause of action — is limited to the amount you paid for the specific transaction giving rise to the claim. MedRebuttal is not liable for any indirect, incidental, consequential, special, or punitive damages arising from your use of or reliance on a generated letter, including any adverse outcome from your insurance appeal.

10. Disclaimer of Warranties

This service is provided "as is" and "as available" without warranty of any kind, express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, accuracy, completeness, or non-infringement. MedRebuttal does not warrant that the letters generated by this tool are legally sufficient, clinically accurate, or appropriate for your specific denial, insurer, or plan type.

11. Governing Law

These terms are governed by applicable law. Any dispute arising from your use of this service shall be resolved through binding arbitration or in a court of competent jurisdiction, as permitted by applicable law.

12. Changes to These Terms

We reserve the right to modify these terms at any time. Changes will be posted on this page with an updated date. Your continued use of this service after changes are posted constitutes your acceptance of the revised terms.
